Urgent Root Canal Care at Our W1 Practice
Root canal treatment (endodontic therapy) becomes necessary when the pulp, the soft tissue of nerves and blood vessels inside a tooth, is infected or irreversibly inflamed, often following deep decay, a crack or trauma. Left untreated, the infection can progress to an abscess, bone loss and ultimately loss of the tooth.
At Wimpole Street Medical & Dental in Marylebone, our emergency dentists carry out root canal treatment to ease pain and preserve the natural tooth. For patients in acute pain we aim to begin treatment the same day, subject to availability, including emergency pulp extirpation (nerve removal) for prompt relief.
We're open 6 days a week, with late evenings on Tuesdays and Thursdays until 8pm plus Saturday hours. Our clinic at 22 Wimpole Street is a short walk from Bond Street and Oxford Circus stations.

What Happens During Root Canal Treatment
Knowing what to expect can take the anxiety out of treatment. Here's how it unfolds, step by step.
Emergency Assessment & X-Ray
We examine the tooth and take a digital X-ray to assess the extent of infection or damage to the nerve. We perform vitality tests to determine whether the pulp is still alive.
Local Anaesthesia
The tooth is thoroughly numbed with local anaesthetic. We take care to help ensure the area is fully numb before proceeding, even when infection is present, which can make numbing more challenging.
Pulp Removal (Extirpation)
An access cavity is made in the top of the tooth, and the infected or damaged pulp tissue is carefully removed from the canal system. This step often helps relieve the acute pain.
Canal Cleaning & Shaping
The canal system is cleaned, shaped, and disinfected using specialised instruments and antimicrobial irrigants to help remove bacteria and debris thoroughly.
Canal Filling & Seal
Once the canals are clean, they are filled with a biocompatible material (gutta-percha) and sealed. This aims to prevent bacteria from re-entering the canal system.
Crown Recommendation
After root canal treatment, the tooth is more brittle as it no longer has a blood supply. A dental crown is usually recommended to protect the remaining tooth structure and restore full function.

Myth: "Root canal treatment is extremely painful"

The Challenge
The patient presented with a front tooth (UL1) fractured almost halfway, extremely sensitive and painful. The adjacent tooth (UR1) had a minor chip. Dental trauma to the front teeth is particularly challenging as treatment must restore both strength and natural aesthetics.
The Treatment
Root canal treatment was performed using the Dentsply Sirona X-Smart Pro Plus endodontic motor, followed by fibre post placement and a custom crown. The adjacent tooth was repaired with composite edge bonding. Both incisors were subtly reshaped for a balanced smile.
The Result
The fractured tooth was fully strengthened and protected. The chipped tooth was seamlessly repaired. The patient left pain-free and delighted with a balanced, natural-looking smile.
