Myotonic Dystrophy Type 2 (DM2), also known as Proximal Myotonic Myopathy (PROMM), is caused by an expansion of CCTG tetranucleotide repeats in the CNBP (ZNF9) gene on chromosome 3q. This PCR-based test detects the pathogenic repeat expansion. DM2 is generally milder than DM1 and does not show significant anticipation.
Who Should Consider DM2 ZNF9 Repeat Testing?
Individuals with proximal muscle weakness, myotonia, muscle pain, cataracts, or cardiac conduction abnormalities where DM1 testing has been negative. Those with a family history of DM2 seeking predictive testing. DM2 typically presents later in life (third to sixth decade) with milder symptoms than DM1.
Normal alleles have up to 26 CCTG repeats. Pathogenic expansions range from approximately 75 to over 11,000 repeats. Unlike DM1, there is no congenital form of DM2 and the condition does not show significant anticipation. Genetic counselling is recommended to discuss inheritance and implications for family members.
